At Full Circle Therapy and Wellness, we know you may have questions about our services, appointments, or what to expect during your visit. Our FAQ section is here to help you find quick, clear answers about how we work, our approach to care, and how we can best support your health and wellness journey.

What does a physical therapy evaluation consist of?

You will be evaluated by a licensed physical therapist for approximately 60 minutes. During that time we will discuss your areas of concern, mechanism of injury, assess your strength and movement patterns, and identify the local and root causes of your issues. Then together we will develop a treatment plan that will restore your mobility, increase your strength, eliminate your pain and achieve all your physical health goals. 

For best assessment and treatment options, you should come dressed in loose-fitting, light weight clothing. Tight fitting clothing, even sports bras and yoga pants, can impede motion that we need to assess. We will have hospital gowns and athletic shorts on hand if needed.

We are In-Network with most of the common insurance carriers. Please contact our office if you have specific questions. We do also have cash-pay options at very competitive rates so that we can assist you in achieving your goals without insurance barriers.

All Medicaid plans REQUIRE a referral. 

All Medicare plans SUGGEST a referral, and your physician will need to sign the plan of care that is set by the evaluating physical therapist after evaluation. 

Most other insurances do not require a referral. Please call your insurance company if you have specific questions about what they cover. 

After your initial evaluation your physical therapist can provide a diagnosis Code and CPT codes if you want to check your benefits.
We are able to do telehealth sessions, although certain insurance companies do not cover this service. Please contact our office if you have specific questions.
Your plan of care will be decided on the day of your evaluation. Most appointments are 1-3x/week and wean down in frequency as your needs change.
Physical therapists cannot prescribe medication, but we can suggest supplements that may assist your recovery. Please always check with your medical provider to check if these supplements will be right for your body.

Full Circle Therapy and Wellness is located at 1325 S. Broadway, De Pere, WI 54208. We are across the street from the Swan Club, and we share a driveway with Starbucks on South Broadway. Go left in the driveway, and there is parking in front of our building.
